Ventilator-Dependent Care
Oak Summit is the only long-term care nursing facility in Forsyth County offering ventilator-dependent care. Appropriate for those residents who are ventilator-dependent and non-weanable, expert care is provided on a separate 10-bed unit by nurses and respiratory therapists who specialize in this area.

When ventilator care residents are able to transition to home, Oak Summit assures that their home healthcare will be superior as well through in-depth discharge planning.

Long-Term Care
Long-term care patients require a safe environment and constant reassurance. At Oak Summit, there are 24-hour registered nurses and an interdisciplinary team of professionals that plan each resident’s care. Some of our long-term care residents are transfered to a lower level of care or are discharged to home.

For those residents suffering from dementia or Alzheimer’s Disease, a special wandering alarm system guarantees their well-being while allowing them greater mobility and independence.

Rehabilitative/Short-Term Care
Through the rehabilitative/short-term care program, residents who are discharged from the hospital or other facilities receive the medical attention, personal support and rehabilitative physical, occupational or speech therapy they need to return home.

The program provides rehabilitative services and strengthening to patients who have had a recent hospital stay because of stroke, orthopedic surgery, neurological problems or other unexpected illnesses or injuries.

Because of the increasing need for these services, Oak Summit has doubled the number of rehabilitative/short-term care beds available to the community.

Continuum of Care
Transitioning from a hospital setting into long- or short-term care can sometimes be stressful. Oak Summit offers residents and their families a continuum of care environment through close communication between our Admissions Coordinator and referring agencies. This approach allows us to appropriately address the emotional, physical and spiritual needs of each resident.

Collaborative Resident Care
Resident care is planned and provided by an interdisciplinary team of on-site geriatricians, specially trained nurses, and dedicated respiratory, physical, occupational and speech therapists who contribute specialized knowledge to the rehabilitation and care of each patient.

Through weekly and monthly meetings, the care team develops individualized care plans for residents and monitors their progress.

Alzheimer's Care
Oak Summit offers a specialized program for residents with Alzheimer’s Disease or dementia. Each patient is evaluated by the interdisciplinary team, and an individual care plan is developed. The care plan focuses on the strongest memory systems so that residents can compensate for weaker areas. Through speech therapy, occupational therapy and specially designed activities, the care team helps residents communicate and accomplish daily activities such as grooming, dressing and eating.

Recreational opportunities such as putt-putt golf, gardening and cooking are planned to help facilitate recognition and communication.

Oak Summit is a restraint-free facility. A wandering alarm system provides safety to residents while providing greater mobility and independence.

Respite Care
Respite care is offered in response to a growing demand for assistance to family members/caregivers. The primary objective of respite care is to provide the regular caregiver with temporary psychological and physical relief from the responsibility of constant care of their loved one. Respite care is a proven cost effective means of delaying and/or eliminating the need for permanent institutionalization of the elderly. Residents are provided a room, meals and nursing care including assistance with daily activities. Medicines or additional care items should be provided by the person or family seeking the respite care.

OTHER SUPPORT SERVICES

Physical Therapy
Residents may receive physical therapy five times a week by on-site physical therapists in a fully-equipped gym. Physical therapy is typically provided to residents who have suffered a stroke or trauma, or who are impaired due to orthopedic surgery, muscular skeletal injury, cancer, amputation or neurological event.

Each resident benefits from an individualized therapy plan that measures progress in short-term gait, transfers, physical endurance and safety.

Because of the superior rehabilitative services at Oak Summit, resident outcome statistics are very high (INSERT OUTCOME STATISTICS).

Occupational Therapy
Occupational therapists work with each resident to increase his or her independence in daily activities such as bathing, dressing and grooming. Residents learn to cook for themselves in our fully equipped kitchen, as well as how to maintain a checkbook, make an appointment or react in an emergency. The program also provides low vision accommodations.

Speech Therapy
Oak Summit’s on-site speech therapist evaluates residents for speech and swallowing disorders caused by neurological incidents, trauma or cognitive deficits. Speech therapists conduct sessions three to five times per week, utilizing a variety of treatments such as electrical stimulation, deep pharyngeal nerve stimulation or vitastem therapy to strengthen the swallowing response. For residents who experience facial droop, neuromuscular stimulation to strengthen facial muscles is important for appearance, speech and swallowing.

The Speech Therapist works collaboratively with other care providers to increase communication skills of those residents with Alzheimer’s disease or dementia.

Wound and Skin Care
The interdisciplinary wound and skin care team consists of a geriatrician, wound-care nurse, nutrition specialist and pharmacist who employ a variety of therapeutic options in the treatment of diabetic, surgical, pressure or trauma wounds.

The team also provides comprehensive wound prevention care for all residents.

Pharmacy
Oak Summit utilizes Baptist Home Care Pharmacy with on-site pharmacists who specialize in geriatric medicine.

Nutritional Consultation
Nutrition plays an integral part in recuperating, gaining strength and staying well. Proper dietary choices and selections influence a resident’s comfort, security and feeling at home. Oak Summit dietary specialists serve as personal consultants to assure that nutritional and emotional needs are met.
